Associate/Affiliation Fee is Fifty Dollars ($50.00).
Dues for an Associate are $115 (Mecca Dues only as your Per Capita and Hospital Assessment are paid at your home temple). 
Dues for an Affiliation are a total of $150 ($115 Mecca Dues, $30 Per Capita, $5 Hospital Assessment). You must have a certificate of Demit in good standing to be voted on as an Affiliation memebership.
